The formal and secure collection of personal, medical, and financial information necessary for patient intake and scheduling is strictly handled off-website through a separate, secure Electronic Health Record (EHR) system.
- The Website may direct users to a separate, secure EHR platform operated by or on behalf of independent providers. This platform operates independently from the Website and is governed by its own privacy and security policies.
- All clinical intake forms, consent documents, and financial agreements are hosted and processed exclusively within the secure, HIPAA-compliant EHR platform utilized by the independent providers who practice on the Agape platform.

Agape serves only as a management and administrative services organization (MSO) to independent mental health providers.
- The independent providers retain full authority over all clinical decisions, diagnosis, and treatment.
- The independent providers are solely responsible for the PHI collected during the intake and treatment process via the secure EHR system, subject to the Notice of Privacy Practices they provide to their patients.
- No Provider-Patient Relationship
Use of this Website does not establish a provider-patient or client relationship between you and Agape Mental Health, LLC or any independent provider. Any such relationship is established only through formal intake and consent processes completed within the secure clinical system.
